Frequently asked questions

How do I enroll in services?

Simply call us at 317.887.3290 x0 and speak to one of our specialists to schedule an appointment for a confidential assessment. During the assessment, you will meet with a therapist to discuss your particular situation. Based on the information gathered at that time, an appropriate level of treatment will be recommended. If you're wanting to enroll in Domestic Violence services, simply bring any legal and/or referral paperwork with you (if applicable) to one of the DV groups listed on our group schedule. We will meet with you to fill out the enrollment paperwork immediately following your first group.

What do I need to bring with me to my initial appointment?

Please bring the following to your initial appointment:

  • Photo I.D.
  • Initial assessment fee
  • Last two pay stubs (if wishing to apply for sliding-fee scale)
  • Any referral paperwork (if applicable)
  • Any legal paperwork (if legally referred)

What program or service is right for me?

This question will be answered at the completion of the clinical assessment. Life Recovery Center offers several types of outpatient treatment. If there is no treatment that is appropriate for your particular situation offered at Life Recovery Center, the therapist will refer you to another agency that can meet your needs. Sometimes treatment is not necessary.

How much will it cost?

That depends on the service you're receiving, but we can assure you that our rates are affordable. Our goal has always been to offer services to all individuals, regardless of their financial situation. We offer a variety of financial and funding assistance options, meeting our clients with rates that they can reasonably afford based on their family size and income. Special financial assistance is available for individuals who qualify through various programs. We will determine your eligibility for this financial help at your initial appointment. Some of our services are also reimbursable through most insurance plans. With self-pay clients, we accept payments by cash, money order, or credit card. No matter what your financial situation, we will make every effort we can to meet your needs. Do you accept insurance?

Yes. We we are a Medicaid/HIP provider and accept health insurance for a number of our services. If you want to use insurance to pay for your treatment at LRC, please complete this form and call prior to your initial appointment to give us your insurance information. We will investigate your benefits to see if your insurance plan will pay for your treatment. We have therapists on staff who are in network with several insurance companies, including

  • Medicaid/HIP
  • Anthem
  • Aetna
  • Sagamore
  • Cigna
  • United Behavioral Health
  • SIHO
Even if your provider is not on this list, most insurance companies offer some level of reimbursement for our services. If you'll give us a call, we are more than happy to give you the necessary information to give your insurance provider to find out about your reimbursement options. Many individuals find that our rates are just as cheap or cheaper than their insurance co-pay and opt not to use their insurance. If you would like to explore this option further, please contact us.

What if I don't have health insurance?

If you are uninsured, this is NOT a problem. We operate on a sliding-fee scale, meeting clients with rates that are affordable. Special financial assistance is available for individuals who qualify through various programs. We will determine your eligibility for this financial help at your initial appointment.

When do the groups meet? What are your operating hours?

We have groups in the morning, mid-day, and evening all week long. We offer groups so often because our goal is to make treatment readily available for everyone, regardless of how busy their schedule. Our administrative office hours are from 8AM-5PM Monday-Friday. You can find more information on our schedule page.

I'm interested in doing individual sessions instead of groups. Can I do that?

We most certainly do. Our therapists are available for individual therapy appointments for issues ranging from substance abuse to depression to anxiety and more. Simply call us at 317.887.3290 x0 and speak to one of our specialists to schedule an initial session. We are able to accept insurance as a form of payment for individual counseling sessions. ABSOLUTELY NOT. Our services are completely confidential. We do not release ANY information about you without your written consent to do so. Our goal is to always protect your right to privacy and your confidentiality.
